Responsibilities

The Health Home Care Coordinator will work directly with children and youth diagnosed with severe and chronic medical conditions in order to increase access to appropriate behavioral and physical health services, reduce emergency room visits and prevent medical crises whenever possible. This position performs care coordination duties for children who are high-cost, high need and presenting with co-occurring serious and chronic medical conditions and behavioral health conditions.  The Lead Health Home Care Coordinator provides HH team leadership to ensure Health Home staff’s completion of necessary medical and mental health assessments with entry of medical documentation into an electronic health record, assists team of health home care coordinators in their support of child patients in the identification of individual medical goals, teams with medical professionals and specialists to support a coordinated plan of care and works to ensure the success of that plan.

Qualifications

Requirements

  • Bachelor's degree in a related medical or mental health field with two years’ of experience.
  • Experience in working with individuals with co-morbid chronic condition management as well as behavioral change methods
  • 1 year experience providing behavioral or medical clinical or care coordination to children and their families.
