    Family Housing Network provides a place to live for families experiencing or at risk of homelessness, while helping to build skills that maximize family stability and self-sufficiency. Through FHN the Veteran Homeless Prevention program provides supportive services to Veteran households at imminent risk of homelessness, or who have become homeless within the past 90 days. Homeless Adult Services provides 24/7 services to homeless individuals in Pierce County which includes meals, shelter, community socialization and support services.

     

    The Supportive Services for Veteran Families (SSVF) Lead Case Manager is a member of the RRH service team for Family Housing Network, a housing program of Catholic Community Services. This position supports the SSVF Supervisor & Rapid Re-Housing Program Manager, in problem solving, resource management, and reviewing and auditing case files for ongoing quality assurance and training purposes. The lead SSVF case manager works closely with the SSVF Supervisor & Rapid Re-Housing Program Manager and the community at large, he/she also provides support as requested with the overall objective to provide outstanding case management to all families in Family Housing Network Rapid Re-Housing programs. This position also maintains a caseload of families in community housing. This includes developing and implementing client-defined, goal-oriented Housing Stability Plans, assessing the needs of homeless families and children, utilizing resources to meet identified individual needs with the ultimate goal of obtaining and keeping permanent housing.

     

    The job will branch into the surrounding counties: Thurston, Kitsap, Mason, Lewis, Gray's Harbor, Waikakum and Cowlitz

    Responsibilities

    Assists with training and oversight to Rapid Re-Housing teams while promoting a cooperative, team environment.  Support, teach, FHN team mates to ensure ongoing growth in skill level and experience working with issues impacting homeless families with children and high quality case notes and case files.

    Employee accomplishes this responsibility by completing the following tasks:

    • Assists with scheduling and performing with fellow case managers, periodic review of case files, case notes, and other pertinent information for quality assurance, compliance with funding contract requirements.
    • Collaborates with Program Manager and SSVF Supervisor concerning critical program information, community resources, case management training needs, and other information to assist in managing the programs of Family Housing Network.
    • Assists the SSVF Supervisor with the development of FHN case management standards, guidelines and paperwork to assure high quality care management to FHN SSVF households.
    • Actively participates in weekly case management meetings.
    • HMIS participation: participates in all required HMIS trainings; collaborates with CCS Data Administrator concerning data quality needs specific to Coordinated Entry referrals and assignments.
    • Assists the SSVF Supervisor in tracking and assigning of clients to SSVF case managers.
    • Budget management: actively participates in spending down appropriate funds for SSVF programs; works with supervisor and operations manager to stay within budgets limits and spending needs.
    • Serves as mentor to follow staff members, be available for consultation on client cases as needed, and report pertinent information to the Rapid Re-Housing & Veteran Services Supervisor.

    This position maintains a caseload of Supportive Services for Veteran Families households.

    Employee accomplishes this responsibility by completing the following tasks:

    • Pulls names directly from the Veteran Master List in order assign to SSVF case managers.  Is knowledgeable and up-to-date on referral system requirements.  Participates in Veteran Master List meetings as assigned.
    • Works in collaboration with SSVF staff to provide updates concerning Coordinated Entry referral system.
    • Assist the SSVF Supervisor in monitoring the case management team's service quality and documentation; and assisting with the SSVF team's data correction requests as needed.
    • Performs intake, assessment of family strengths and needs and identify support services appropriate for the immediate needs of the family.
    • Case manages household’s specific to Rapid Re-Housing programs of Family Housing Network.
    • Once housing is located, request required inspection performed by Pierce County Human Services; in addition, serves as communication link with on-site manager.
    • Assist families in developing a Housing Stability Plan and identifying the steps required to accomplish goals to move the family from homeless to self-sufficiency.
    • Develop a client directed HSP according to family needs (education, employment, health care, finances, parenting, etc.)
    • Coordinate with and make referrals to appropriate community agencies.
    • Maintain well organized, up-to-date case files to include but not limited to the following: homeless & income verification- to be aligned with intake dates of eligibility, case notes outline all communication with clients, regular staffing notes reviewed by supervisor, HSP that is reviewed and updated on a regular basis, Habitability/HQS documentation and tenant rent calculations (as required by grant).
    • Participate in weekly case management meetings in collaboration with all FHN SSVF programs

    Provide direct services to families as needed
    Employee accomplishes this responsibility by completing the following tasks:

    • Provides active case management support to FHN SSVF families' to include but not limited to meetin households in their homes, providing transportation for appointments as needed, active referrals to community resources, employment and education referrals as well as housin location. Facilitate contact and appointment for community resources, financial assistance, medical care, social services, and any other needed services.
    • Maintain weekly contact with all caseload families.
    • Meet at lease semi-monthly with families to monitor goals and update case plan.
    • Provide crisis intervention with clients as necessary.

    Provide records and reports.

    Employee accomplishes this responsibility by completing the following tasks:

    • Keep accurate and timely case management and staffing notes.
    • Keep organized and confidential records.
    • Keep timely and accurate HMIS data information to include referrals, financial information and mandatory data elements.
    • Participate in case management meetings and provide case summaries and client status information.
    • Prepare monthly report to include case summaries and statistical information.

    Attend meetings as necessary and represent Family Housing Network.

    Employee accomplishes this responsibility by completing the following tasks:

    • Attend meetings as required by SSVF Supervisor or Program Manager to include but not limited to RRH collaborative, PC Provider meeting and Coordinated Entry Referral et al meetings
    • Attend appropriate training workshops and opportunities to continually improve knowledge and skills.
    • Share appropriate information with other staff.

    Qualifications

    Minimum Qualifications

    • Post-secondary education in Human Service/Social Service field
    • Minimum 2 years relevant case management/housing experience.
    • Experience with Rapid Re-Housing, landlord/tenant law, fair housing and other housing issues.
    • Experience providing services to low-income/no-income families.
    • Ability and willingness to work in a team environment and promote a positive team spirit.
    • Excellent inter-personnel skills including excellent oral and written communication skills
    • Excellent organizational and time management skills
    • Ability to work with a diverse spectrum of high-need individuals and maintain healthy boundaries.
    • Willingness to meet in individual family homes
    • Valid Washington State driver’s license, automobile, and insurance

    Preferred Qualifications

    • Bachelors or Master’s Degree in Social Services
    • Knowledge of Pierce County social services and other community resources.
    • Strong computer skills including Microsoft Word and database experience
    • Bilingual skills (English-Spanish)
    • Grant writing and reporting skills and experience
