  • Overview

    Bakhita Gardens provides safe and comfortable dwellings for a diverse community of women, particularly those most vulnerable.  Three programs exist in the Bakhita Gardens building: Noel House Enhanced Shelter: 20 semi-private shelter units where homeless women can stay for up to a year while looking for permanent housing; Noel House Permanent Housing: 20 semi-private permanent housing units designed for women who have been homeless the longest and who have the most difficulty navigating systems; Rose of Lima House: 50 single-room occupancy units of Permanent Supportive Housing for women moving out of homelessness. Bakhita Gardens fosters a supportive, team-oriented work environment where each staff provides advocacy, information and referrals to the women using our services. 



    This position is responsible for case management at Noel House Enhanced Shelter, including client assessment, referrals and other duties as outlined below.  This position is responsible for developing housing resources and relationships with housing agencies and landlords in an effort to match client with appropriate housing opportunities.



    Client Engagement & Assistance:

    1. Intake and development of individualized goal plans unique to each client’s needs.
    2. Meet monthly with clients to evaluate progress and update goal plans, with a focus on housing.
    3. Identify needs and barriers to housing and make appropriate referrals to resources in the community.
    4. Actively provide outreach and engagement for hard-to-serve clients.
    5. Initiate and participate in care conferences with clients and NHP supervisors as needed.
    6. Assist clients with filling out the necessary paperwork, including benefit and housing applications.
    7. Assist with gathering necessary documentation, including homeless verification, disability verification, and other documentation needs.
    8. Accompany clients to various appointments, interviews and housing searches as needed.
    9. Arrange housing tours to permanent and transitional housing programs.
    10. Secure financial move-in assistance for clients moving into permanent housing.
    11. Facilitate monthly community meetings to elicit feedback on service delivery and policies/procedures.
    12. Provide guidance for and evaluation of MSW practicum students.


    Housing Outreach & Coordinated Entry:

    1. Manage Coordinated Entry for All (CEA) process for Noel House, including performing client assessments; identifying clients for housing placements; and assisting colleagues with the implementation of Coordinated Entry.
    2. Act as the point person for CEA housing referrals for Noel House shelter.
    3. Participate in weekly CEA case conferencing meeting, which includes advocating for Noel House clients to be considered for available permanent supportive housing units.
    4. As part of NHP Case Management team, identify and outreach homeless women to move into Noel House Programs and other set-aside housing units within Catholic Housing Services.
    5. Build a network of housing resources, both public and private, that best meet the needs of single adults.
    6. Meet with landlords to share NHP’s mission and advocate directly for clients seeking housing.



    Administrative & Program Support Duties:

    1. Maintain and review case records and documentation in files and log.
    2. Maintain case management statistics in compliance with HMIS.
    3. Maintain resources in files available to other staff and residents.
    4. Complete quarterly/yearly demographics reports for government and private funders.
    5. Participate in required staff meetings as scheduled, including Client Meetings, Admin Meetings, Case Management meetings, and All Staff meetings.
    6. Provide information to NH staff around relevant case management issues and best practices.
    7. Represent NH in service provider networks.
    8. Assist in the supervision of program volunteers and student interns.
    9. Develop and safe-keep a workplace that values and supports a culturally diverse work environment.


    General Responsibilities

    1. Maintain accurate record of hours worked and turn in timesheets on schedule.
    2. Attend trainings required of employees.
    3. Maintain confidentiality of program, staff and clients.
    4. Maintain cooperative relationships with residents/businesses in surrounding neighborhood.
    5. Contribute to the mission of Catholic Community Services and Catholic Housing Services.
    6. Contribute to and support a positive, team-oriented work environment.
    7. Perform other job-related duties as assigned.


    1. Bachelor’s degree in human services or social work field (see Substitute Qualifications)
    2. Two years of direct experience in social service programs, including knowledge of women’s issues, homelessness, mental illness and addictions.
    3. Understanding of harm reduction and housing first service models.
    4. Experience working with and understanding culturally diverse groups.
    5. Demonstrable problem-solving/issue assessment skills, intervention planning/implementation skills, and/or crisis intervention skills.
    6. Excellent oral and written communication skills, team-building skills. Knowledge of Word, Excel, and basic internet use.
    7. Ability to uphold/model the mission, values, and insights of Catholic Community Services and Catholic Housing Services in all aspects of work life and to uphold the mission and values of Noel House.


    SUBSTITUTE QUALIFICATIONS: (Qualifications which may be substituted for minimum qualifications.)

    • A combination of education/training, and demonstrated knowledge/skills and experience, may in some cases substitute for the BA requirement.
