Program Description

New Bethlehem Place provides 24/7 enhanced shelter, day center, and supportive services to families experiencing homelessness.  


Position Description

The Case Manager is a key member of the NBP team and serves as the primary director of care and connection to resources and housing for families at NBP. The Case Managers coordinate intakes and case management services for New Bethlehem Place 24/7 Shelter and Day Center programs. The position is responsible for year-round case management, and will coordinate with the Lead advocate, and services advocate team to make sure that clients are well served. The Case Manager will spend at least one evening a week connecting with clients in addition to day time hours. 

Responsibilities

Provide screening and intake, as well as case management, for NBP’s program participants. 

  • Conduct initial intake of families seeking shelter.
  • Treat program participants in a culturally sensitive manner and actively seek to become informed on issues where culturally-determined perceptions might be different. 
  • Meet with program participants of the NBP shelter and day center programs in person weekly to work on goals and plan for the future.  
  • Work primarily on goals related to obtaining housing and increasing incomes. 
  • Work with program participants to develop plans for stability, including the areas of housing, job search, drug and alcohol issues, physical and mental health, personal development, and other life domains. 
  • Provide referrals and advocate for program participants with other social service agencies, hospitals, mental health agencies, and treatment centers. 
  • Meet with program participants to assess employment options. 
  • Provide follow up supportive services to program participants working toward employment goals. 
  • Monitor the emergent needs of program participants and provides resource assistance. 
  • With Program Manager, develop contacts with local businesses that employ program participants and makes referrals. 
  • Conduct follow-up for the NBP shelters programs’ participants once they have moved on, from the program, ensuring quality of care. 
  • Work with Program Manager and Operations Manager to ensure good communication with clients and volunteers. 
  • Complete assessments on new clients and connect them to the King County Coordinated Entry system. 
  • Engage clients in other CCS programs such as RRH, Keeping Families Together, and CREW. 

 

Represent and promote Catholic Community Services, New Bethlehem Place Shelter programs in the community, strengthening relationships with area faith-based organizations, volunteers, and other community partners. Support strategy, collaboration, and coordination that enhance the mission of Catholic Community Services, New Bethlehem Place program. 

  • Maintain good relations with volunteers. 
  • Represent the interests of NBP program by giving presentations and attending meetings in the community, such as DSHS committees, CEA, and CCS case management working group. 
  • Work well with and collaborate with case management of other organizations occupying the Kirkland Shelter for Families and Women, including the Sophia Way and CCS’ CREW team.
  • Conduct outreach to sites where people who are homeless frequent, including regular outreach to Lake Washington United Methodist’s Safe Parking Program.

 

Develop proficiency in maintaining Case Notes using CCS program participant database.  

  • Data entry – NBP shelter program. 
  • Development of NBP Case Notes as well as oversight of NBP Shelter electronic log entries  
  • Regularly review NBP shelter Case Notes to ensure accuracy  
  • Creation and maintenance of a paperless case file on each participant. 

 

Maintain program participant files according to Quality Assurance standards.  

  • Maintain files and ensure all necessary documentation is included. 
  • Conduct quarterly reviews of program participant file case notes. 
  • Ensure files are in compliance with agency and contract requirements. 

Qualifications

  • Associate's degree in social services or equivalent experience. 
  • Two (2) years direct experience with homeless and/or at-risk populations. 
  • At least one (1) years of case management or equivalent experience. 
  • Knowledge of social services resources and programs in King County. 
  • Ability to enforce shelters’ rules, including maintaining a drug and alcohol-free shelters. 
  • Ability to work with culturally diverse clients. 
  • Ability to work independently and with a team. 
  • Computer skills and ability to learn and use CCS client database system. 
  • Valid WA State Driver’s License and auto insurance, and ability to meet CCS Driving Policy requirements. 
  • CPR/First Aid Certificates/Blood Borne Pathogens - within 30 days of hire. 
  • Food Handlers Permit - within 30 days of hire.  
  • Current TB test, or willing to obtain within 30 days of hire.

  • Demonstrate the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
  • Applicant must successfully pass required background clearances prior to an offer of employment.

Preferred Qualifications  

  • Familiarity with criminal justice system and/or issues involved with drug/alcohol abuse.
