Housing and Essential Needs is a statewide program. Catholic Community Services has contracted with Thurston County and works with the Washington State Department of Commerce to implement and deliver Housing and Essential Needs benefits in Thurston County. DSHS clients enrolled in Medical Care Services (MCS) and ABD are eligible to receive funding to help cover rental and/or utility expenses, get temporary hotel stays, obtain essential items, etc.

 

The Outreach Case Manager works in collaboration with the rest of the CCS HEN team. The Outreach Case Manager completes HEN eligibility intakes at primary office as well as in various areas of Thurston County. This position helps establish sites where intakes can be conducted so that all applicants do not have to come to the main office. This position also provides Case Management and Crisis Intervention services for identified clients who are having difficulty finding housing, employment, and other resources. This position helps current clients find resources that they can use to transition off the HEN Program. This position helps keep excellent records and helps to assure Consolidated Homeless Grant compliance.

Responsibilities

MAJOR DUTIES AND RESPONSIBLITIES:

  • Completes intake for the HEN Program.
  • Coordinate with the local Coordinated Entry provider to assure that clients are screened and placed into a priority pool based on vulnerability.
  • Answer telephone and conduct phone interviews to gather information regarding eligibility for the HEN Program.
  • Provide Case Management services for vulnerable clients who have difficulty accessing housing and resources
  • Create client directed service plans.
  • Transport clients to visit housing opportunities.
  • Advocate on behalf of clients.
  • Assist clients in connecting to resources that help them overcome barriers.
  • Provide HEN Intakes.
  • Provide intakes in the main office.
  • Establish other sites within Thurston County to conduct intakes in order create easier access to HEN benefits for those who do not reside near the Main office.
  • Ensure strict compliance with all CHG guidelines.
  • Work to establish connections with resources including prospective landlords.
  • Prepare and organize client files, mail out information and documents to clients and vendors.
  • Maintain excellent and complete client files with timely case notes, and documentation.
  • Data entry and filing of confidential records.
  • Provide follow up with clients who are transitioning or who have already transitioned in order to make sure outcomes are recorded accurately.
  • Input data into HMIS in a timely manner.
  • Assist in maintaining forms, supplies and essential needs items.
  • Applicant must successfully pass required background clearances prior to an offer of employment.
  • Ability to work both independently and as a member of a team.
  • Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services.
  • Demonstrate the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.

 

Crisis Intervention

  • Ensure services are provided in a setting that provides for the safety of the individual and agency staff members.
  • If an individual is in crisis, remain with them in order to provide stabilization and support until the crisis is resolved or a referral to another service is accomplished.
  • Assure communication and coordination with the individual's mental health care provider, if indicated and appropriate.
  • Document each telephone and in-person crisis response.
  • Crisis telephone support as needed.
  • Crisis outreach services as needed.

Qualifications

Minimum Qualifications

 

  • AA Degree in Social Service or related field
  • At least one year of experience in direct provision of social services, preferably with homeless people.
  • Applicant must successfully pass required background checks prior to an offer of employment.
  • Understanding of problems homeless people often face, including alcohol and drug abuse, mental illness, domestic violence, and long-term homelessness.
  • Excellent written and oral communication.
  • Good office computer skills, with ability to maintain complex client records and update databases including HMIS.
  • Support and contribute to a creative, collaborative and respectful environment that promotes teamwork.
  • Mental health and/or crisis intervention experience.
  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations

Preferred Qualifications

  • Bachelor of Master’s Degree in Social Work or related field
