Program Description: Southwest Adult Behavioral Health serves individuals who are Medicaid eligible, providing mental health services and Peer Navigator supports.  The Foundational Community Supports program offers supportive housing services through systematic outreach and clinical intervention for individuals who receive services through HAS, CCS programs, and the wider community.  

 

This position provides day to day guidance, knowledge and support to the Foundational Community Supports teams across Southwest counties. This position involves direct technical assistance, collaboration, and training to sites offering FCS services in their location. This position collaborates closely with the Site Directors and the SW Clinical Director to ensure contract compliance, adherence to WAC and other regulations, MCO requirements and service delivery.

Responsibilities

MAJOR DUTIES AND RESPONSIBILITIES

  • Collaborate with Site Directors to support operations of Foundational Community Supports.
  • Works with MHP and Regional Director to monitor systems to ensure compliance with all Foundational Community Support contract requirements and regulations, including documentation compliance.
  • Support site directors to train Certified Peer Specialists who provide Foundational Community Supports
  • Assist in file reviews for Foundational Community Supports, clinical documentation in Care Logic EHR, and billing.
  • Complete FCS Supportive Housing assessments and Housing Plans as-needed.
  • Provide services consistent with FCS contract requirements, program and agency policy, and CCS service philosophies.
  • Adheres to the implementation of policies and procedures that comply with funder requirements and applicable laws and regulations.
  • Ensures client records are kept in accordance with WAC, agency standards, and contract requirements.
  • Maintain a system to document TAP, and HRSN funding requests as needed.
  • Ensures timely and accurate reports as required by contracts.
  • Ensures data entry into the local data entry systems occurs as needed.
  • Maintain availability to respond immediately to staff and site leadership in crisis situations.
  • Supports leadership in creating a culture of privacy and security of Protected Health Information in HIPAA Covered Entity service area, including education of staff about their responsibilities for safeguarding privacy and security.

Contract Compliance

  • Supports Site Directors and Regional Director of SW Behavioral Health with Foundational Community Support issues, adherence to WAC requirements, DBHR, MCO, quality assurance, etc.
  • Ensure that clients receive an FCS assessment as outlined in Access Standard and that authorization for routine services beyond evaluation and assessment is based on Medical Necessity.
  • Ensure ongoing compliance with FCS and other entities regarding contract requirements and any site reviews and audits.
  • Monitors and maintains HMIS/Clarity, Availity, Smartsheet.

Community Involvement

  • Supports and maintains strong working relationships with departments within Nativity House Shelter and other CCS programs, community partners, volunteers, and other constituencies.
  • Provide outreach and education to community providers, pursuing service expansion opportunities as appropriate.
  • Engage with local housing initiatives including Coordinated Entry Projects.
  • Attend meetings (i.e., QI, budget, CareLogic, community and county meetings, housing meetings), representing the FCS Supportive Housing and CCS in a professional manner.

Fiscal

  • Assist Site Director in monitoring costs, associated with the FCS programing.
  • Confirm FCS and/or other funders receive documented completion of deliverables per contract requirements and timeframes.
  • Keep an accurate ledger of requests and expenses relating to temporary financial assistance provided to clients via TAP and HRSN funds when available.

Training

  • Provides guidance and training to certified peer counselors providing FCS services.
  • Coach employees to build insight and awareness of self in relationship to client care, recognizing and managing power dynamics through self-reflection. Coach staff in providing culturally competent interventions to ensure equality of treatments.
  • Supports program leadership to ensure workplace safety.
  • Promote ongoing training to include attendance at all CCS mandatory trainings for site employees.
  • Supports program leadership to maintain a culture of privacy and security of Protected Health Information in HIPAA Covered Entity service area, including education of staff about their responsibilities for safeguarding privacy and security.

Client-Facing Duties

  • Maintains a half caseload of ongoing clients for services as needed.
  • Provides back up FCS services for clients should their peer no longer be able to meet with them or provide services until a replacement is found.
  • Completes and maintains all clinical documentation via Care Logic in accordance with WAC, RCW, and CCS policy.
  • Other duties as assigned.

Qualifications

Required Qualifications

  • Must have two (2) years’ experience working under the supervision of a Mental Health Professional.
  • Must have two (2) years’ experience working within a healthcare or integrated care setting.
  • 2 years of direct service experience to at risk populations, including, but not limited to, homelessness, chronic health conditions, substance use disorder, severe mental illness.
  • Exemplary skills in maintaining ‘golden thread’ throughout all clinical and client related documentation.
  • An understanding of, or experience with the provision of services and supports through Certified Peer Counselors.
  • Excellent interpersonal and organizational skills with the ability to communicate respectfully with clients, staff, and community partners.
  • Computer skills with experience with Microsoft Word and Excel.
  • Excellent written and oral communication skills.
  • Applicant must successfully pass required background checks prior to an offer of employment.
  • Proof of negative TB test within 12 months.
  • Must have reliable transportation, valid driver’s license and automobile insurance.
  • Must have an acceptable driving record per CCS’ driving policy. (required to provide current driving abstract)

Preferred Qualifications

  • Has two (2) or more years of experience working with individual experiencing homelessness, significant mental illness, substance abuse, and/or co-occurring disorder.
  • Experience and knowledge in fiscal management and leadership.
  • Trained in crisis de-escalation.
  • Peer supervisory experience.
