The primary goals of the Supervisor/Scheduler include the scheduling of client services in an appropriate and timely manner, the evaluation of Home Care Assistants and the maintenance of regular contact with clients to insure superior service delivery and response to client needs and desires. In addition, if the need arises, assure client/HCA matches (both substitute as well as initial HCA’s), are made pursuant to client needs, HCA level of care, and Union Agreement. Also, be responsible for the use, understanding and data base upkeep of home care computer software and associated processes and paperwork flow.

$17.83 - $18.90 HR/DOE   Catholic Community Services, Homeless Adult Services (HAS) is the largest provider of adult homeless services in Tacoma, Washington. Nativity House (NH), a program of HAS, is the central hub to serve homeless adults in the Pierce County region.   It operates 24-hour/7-day a week, offering services (emergency shelter, meals, systematic outreach, Rapid Rehousing, Mental Health and Co-Occurring Disorder treatment, Employment Services, Spiritual Support, 50 units of permanent supportive housing, etc.) to meet the basic needs of individuals experiencing chronic homelessness guests with meeting their goals.   Staff in this position provides supervision, program structure, behavior management and a wide range of basic medical needs services, as well as more advanced social services. This includes helping guests navigate a myriad of social service agencies, forms and interviews. Staff is trained in behavior management, basic case services, as well as Motivational Interviewing and other trainings pertaining to respite.   ***Full time Monday-Friday=11am-7pm**   ***Part time Saturday-Sunday 2pm-10pm***

Registered Nurse African American Elders Program Full Time $30.00 - $36.00      The African American Elders Program serves African American Elders living in Seattle, Shoreline, and South King County. The program identifies frail, isolated elders, and their caregivers, and assist them in accessing and receiving needed social and health services through ongoing care coordination, support and referrals. The nurse addresses the health effects of inequalities in the prevalence of risk factors and chronic conditions of older African Americans.   This position covers a range of services from assessment through teaching, counseling, health coaching and follow-up activities with the medical community and care coordinators.   The AAEP Nurse promotes access to clinical and preventative health services by: 1. Accessing the functional, medical history, cognitive function, medications, health and well-being of the client, family and home environment. 2. Advocating and Coordinating client care with other health and resource providers, family and community members. 3. Assisting clients with health related referrals, obtain insurance, manage medications, access primary, and specialty care, and other referrals which will improve the clients functioning. 4. Eliminating barriers to care. 5. Monitoring health status of clients. 6. Accessing and protecting PHI/ePHI: • Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area) and complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).   All employees working in CCS have the responsibility for developing and safekeeping a workplace which values and supports a culturally diverse work environment. On Call Relief Driver Part Time - Home Delivered Meals Senior Nutrition Program - Pierce County $17.99 - $19.07 per hour DOE   The Senior Nutrition Program is designed to help increase the nutrient intake of older individuals who might not eat adequately, and, through better nutrition, assist them to remain healthy and independent in their communities. This program consists of two meal programs. Home Delivered Meals is one of programs provided.   In the absence of the regular HDM Driver, the On Call HDM Driver provides delivery of food and related services items to homebound seniors. The On Call HDM Driver will start their day in the Lakewood office and deliver to clients in the Pierce county and may sometimes deliver to clients in Lewis county area.
