We do not compromise our values.   The CCS Health Home team provides coordination of care directly with children and youth diagnosed with severe and chronic medical conditions in order to increase access to appropriate behavioral and physical health services, reduce emergency room visits and prevent medical crises whenever possible. This position works flexible schedules in the home and community setting of the youth and families to ensure completion of necessary medical and mental health assessments with entry of medical documentation into an electronic health record, assists children and families in the identification of individual medical goals, teams with medical professionals and specialists to support a coordinated plan of care, and works to ensure the success of that plan.
