We do not compromise our values.    The Psychiatric Care Coordinator (PCC) provides direct assistance to the psychiatrist(s) and psychiatric ARNP(s) for Family Behavioral Health (FBH), and ensures all medical information is correctly entered into the FBH Electronic Health Record (EHR)/patient chart.   The PCC facilitates patient flow, completes telephonic triage for scheduling of psychiatric evaluations and medication monitoring, and follows up with all areas pertaining to prescriptions of psychotropic medications.  This includes the study of changing formularies within multiple Managed Care Organizations (MCOs), gathering of detailed history of prior psychotropic medications prescribed, documenting dosage, frequency, effects and side effects, and entry of such into the EHR.  Frequently the PCC will negotiate with the MCO (insurance) to receive authorization for needed prescriptions.  This employee will also interact with the MCO Pharmacy Chief (or other applicable MCO authority) when problematic issues pertaining to prescription payment authorizations arise.

They represent a consumer orientation and perspective in all internal and external agency activities.

Through FHN the Veteran Homeless Prevention program provides supportive services to Veteran households at imminent risk of homelessness, or who have become homeless within the past 90 days. Homeless Adult Services provides 24/7 services to homeless individuals in Pierce County which includes meals, shelter, community socialization and support services.   The Coordinated Entry Case Manager is a member of the service team for the Homeless Services division of Catholic Community Services. This position provides screening and intake of people seeking homeless services, utilizing assessment and prioritization tools as well as homeless diversion strategies.  Diversion is a brief emergency intervention that provides a combination of direct services which includes engagement of client resources, landlord/host mediation, connections to mainstream services and resources, and housing placement and/or financial assistance to families as needed. The intended result of Diversion is to identify an alternate safe housing arrangement that removes the immediate need for additional homeless services such as emergency shelter, rapid re-rehousing or transitional housing. Clients without a Diversion option are assessed with a prioritization tool by the Coordinated Entry Case Manager for potential referral to a service provider. (Clients with the highest priority vulnerability and barriers are referred first.)   The Coordinated Entry Case Manager will work directly with individuals and families experiencing homelessness. The successful candidate for this position is energetic and non-judgmental and can actively listen, empathize and facilitate problem-solving with clients toward a stable housing plan, while also helping them navigate community resources. The Coordinated Entry Case Manager seeks to bolster persons’ own ability to think for themselves, weigh and consider options, advocate for their own needs, and ultimately act on their own behalf. The Coordinated Entry Case Manager must also be responsive to emergent housing crises.   Duties include coordinating intakes at multiple community locations with partner agency Associated Ministries, assessing housing needs and providing prompt assistance as needed for housing stability options, and assisting clients to secure housing. The case manager also enters client data into the Homeless Management Information System.  In collaboration with the service team, the Case Manager is responsible for developing and tracking client-defined, goal-oriented Housing Stability Plans, assessing the needs of referred households, utilizing community resources to meet identified individual needs with the goal of stabilizing permanent housing in 30 – 60 days. This position works closely with the Case Management Supervisor and provides support as requested with the overall objective to provide outstanding services to families served by Catholic Community Services.

The Safe Parking Case Manager will work an 11 AM to 7 PM schedule.   The Holy Rosary Safe Parking program is a part of the continuum of services provided by Catholic Community Services-Family Housing Network (CCS-FHN) to families, veterans, and individuals experiencing homelessness in Pierce County. This program provides safe overnight parking and supportive services to families and individuals without a home who are living in their vehicles. Each household receives a Coordinated Entry assessment and case management navigation emphasizing the primary goal of securing permanent housing. The Holy Rosary site is staffed 24/7 to maintain security, and parking participants will have access to restrooms, showers and laundry.

Excellent benefit to include paid vacation, sick leave, 13 paid holidays, dental, vision, life insurance, AD&D, pension, 403b, flex-spending, HSA   Under the supervision of the International Foster Care Program Manager, and the Clinical Director this position is part of a service team that provides for the welfare and mental health needs of immigrant and refugee children referred by the Office of Refugee Resettlement (ORR), and placed into foster care.  As Mental Health Specialist, this position will provide therapy services, trainings, assessments and recommendations related to the mental health needs and treatment of children placed in Long-Term Foster Care (LTFC). This is a 20-hour per week position with a client case load that varies from 1 to 12 clients, with the usual average of  6 clients.  The Mental Health Specialist will also assist as a navigator for contracting outside services for children during times of influx, or other situations when back-up is needed.   Some of the therapeutic services are office based, and some are community based to decrease barriers.  The expectation is that driving will occur across counties a few times per week.  The agency will issue laptop and cell phone to assist with community-based services.  Mileage reimbursement is provided.

This position supervises and coordinates activities of volunteers engaged in serving, food preparation and cooking, dishwashing, and enforces our “Clean as you go” policy.  The cook plans meals based on the incoming inventory each day and the food in the freezer/refrigerator to minimize waste and spoilage.  The evening cook is responsible for ensuring that the volunteer groups who come in to cook dinner for our homeless guests know how to use our kitchen equipment correctly and assist them with any questions that they may have about preparing the food. They will ensure that volunteers leave the kitchen clean, make sure that all of the food is put away, wipe down serving and cutting counters including kitchen equipment, and sweep and mop the floor before they leave. They will be responsible for cooking the meal if a volunteer group is not scheduled or does not show up.
