WHEEL operates two low-barrier shelters for women. Many of the participants of this program have chronic and acute mental health issues, addiction issues, or have suffered severe trauma. The program has a loving, come-as-you-are modality. Position Description The Case Manager is directly responsible for providing Case Management service to one of the two WHEEL shelters and its participants who wish to engage in Case Management to assist in their housing search. The Case Manager will be responsible for identifying barriers to clients obtaining permanent housing, and connecting clients to resources to assist in overcoming those barriers. This position will work closely with Catholic Community Services programs such as the Regional Access Points and Rapid Rehousing Teams and other local resources to assist participants to be able to apply for and obtain permanent housing. This position will require working some days and some evenings, typically Monday through Friday.  An office is available on-site at this shelter, which is located on First Hill.

This position maintains a caseload of individuals in the Right of Way Initiative.  This includes receiving referrals from Right of Way Initiative, assessing housing needs, providing prompt financial assistance as needed for housing costs, and assisting clients to secure housing. The case manager actively uses Housing First and progressive engagement while interviewing and working with client household. In addition, the case manager will enter client data into the Homeless Management Information System.  In collaboration with the service team, the Case Manager is responsible for developing and implementing client-defined, goal-oriented Housing Stability Plans, assessing the needs of referred households, utilizing community resources to meet identified individual needs with the goal of stabilizing permanent housing in the shortest time possible. This position works closely with the supervisors and provides support as requested with the overall objective to provide outstanding case management to individuals served by Catholic Community Services.

This position’s work station is off-site from the CCS Kent Family Center office.

$20.53-$21.76 HR DOE  This will be an ON-CALL position. Evenings, and Weekends possible.    Family Visitation Specialist provides family support services by transporting infants and children for visits with their birth families, monitoring and supervising those visits, and documenting observations of that contact.   Foster Care Child Support Aide Services, commonly referred to as Case Aide Services, augment supervision and activity plan for children whose behaviors of developmental needs cannot be manage in a foster home without additional assistance.    Assignments will be made by the child’s CCS Case Manager for Family Visitation and Child Support Aide Services.

King County Coordinated Entry provides the housing referrals for the units.  Thirty-six units serve veterans referred through the VA. Catholic Community Services and Catholic Housing Services collaborate to provide the most enhanced resident experience and supportive housing community. Thea Bowman Apartments offers 24/7 staffing and on-site case management services.   Position Description The Program Director is responsible for daily leadership to the on-site team that includes CCS social services and 24-hour front desk staff; CHS property management/operations staff; and staff from other agencies that provide services such as healthcare or behavioral health services on-site. The Program Director directly supervises the CCS services staff and collaborates with on-site CHS operations staff to create a strong sense of team united behind a common mission.         The Program Director coordinates all third-party supportive services providers, crisis-response and care conferences. The goal is to meet each residents’ unique needs through frequent and effective communication that is adaptable, stability-focused, and trauma-informed.
