This position maintains a caseload of households in the Supportive Services for Veteran Families program. The position includes outreach activities in the community, completing thorough intakes and eligibility screening with referred clients, assessing housing needs and providing financial assistance for housing costs, and assisting clients to secure housing if they are homeless. The Case Manager uses Housing First and Progressive Engagement approaches to engage and work with Veteran households. In addition, the case manager enters client data into the Homeless Management Information System (HMIS). In collaboration with the service team, the Case Manager is responsible for developing and implementing client-defined, goal-oriented Housing Stability Plans, assessing the needs of referred households and utilizing VA and community resources to meet identified individual needs with the main goal of stabilizing in permanent housing in the shortest time possible. The Veteran Benefit Case Manager is a member of the service team for Veterans Services program of Catholic Community Services.

DSHS clients enrolled in Medical Care Services (MCS) and ABD are eligible to receive funding and basic needs to help them cover rental and/or utility expenses, get temporary hotel stays, obtain essential items, etc.   The Outreach Case Manager works in collaboration with the rest of the CCS HEN team.  The Outreach Case Manager completes HEN eligibility intakes at primary office as well as in various areas of Kitsap County.  This position helps establish sites where intakes can be conducted so that all applicants do not have to come to the main office in Bremerton.  This position also provides Case Management services for identified clients who are having difficulty finding housing, employment, and other resources.  This position helps current clients find resources so that they can transition off of the HEN Program.  This position helps keep excellent records and helps to assure Consolidated Homeless Grant compliance.

The South King County SeaTac Shelter is located in a local hotel occupying 99 rooms.   POSITION DESCRIPTION: The Case Manager is responsible for providing case management service to those individuals enrolled in the shelter program. The Case Manager will coordinate screening, intake and case management service at the locations. The Case Manager will work with clients during the day and will also engage with clients during shelter hours.

Program Description: Catholic Community Services’ Counseling, Recovery & Wellness (CReW) Program is a licensed community mental health and substance use disorder treatment program and provider of the King County Integrated Care Network (KC-ICN). Our program focuses on serving adults with mental illness and/or substance use disorders to help improve individuals’ quality of life. Range of services includes diagnostic assessments, individual therapy, peer support, case management, group counseling, skills training, care coordination, medication management, and psychoeducation. CReW clinicians provide services in a variety of locations within Seattle downtown and greater King County, including within Permanent Supportive Housing programs, shelters, day centers. Services offered utilize Harm Reduction and Recovery principles, with an emphasis on building collaborative relationships with the persons served as the first step towards lasting engagement in treatment.   Position Description: The COD Clinical Case Manager works independently and as part of a team to provide outreach, engagement and ongoing case management services to adults with both mental health and substance use disorders, including those that are homeless and have comorbid physical health conditions. Work occurs in a variety of settings, including housing sites, shelters, the streets and other Eastside community locations.

Catholic Community Services and Catholic Housing Services work together in partnership to provide permanent supportive housing to individuals and families who have experienced homelessness in King County.  We believe that everyone deserves a safe, affordable place to call home.  We understand that people experiencing homelessness come to us with complex histories, and that it takes a highly collaborative, creative team to help end the cycle of homelessness permanently.    The Josephinum is a mixed-income building of 221 units in downtown Seattle, with the majority of units set aside to households earning between 0% and 60% of median income.  The building is home to an array of residents, some with supportive services, others living independently.  The building is also home to Christ Our Hope Catholic Church, a downtown parish that has offices and a chapel.  The Church conducts regular services, sponsors other activities, and has developed considerable relationships between its staff and parishioners and the residents at the Josephinum.   The Josephinum team works from a multi-disciplinary approach in which all staff coordinate services to promote housing stability in an environment that honors and respects the dignity of each resident. We offer compassionate support that emphasizes safety and stability while encouraging sustained growth in independent living skills. We do so in an environment of promise that welcomes diversity and honors each resident's unique experience.   The Program Manager must be a person who is interested in working in a service-enriched housing environment with the goal of enhanced quality of life for the women we serve.    POSITION DESCRIPTION: This position supports the Division Director in overseeing the daily operations of the Josephinum. The Program Manager provides leadership and vision to the Services Team, directly supervising the Resident Services Manager and the Case Management team. Following best practices in serving individuals with chronic histories of homelessness, CHS has developed several Permanent Supportive Housing Communities that follow a Housing First, Harm Reduction based model of services   POSITION DESCRIPTION: Property/Program description Property Managers at each site oversee the property management activities on site, and coordinate with the maintenance team for service to their buildings.  This is a full -time position that provides administrative functions at the location(s) described above. The Property Manager is responsible for the day-to-day administrative operations, including the lease-up of new tenants and recertification of current tenants, overseeing the turnover of units, maintenance and building operations, processing accounts payables, accepting rent, and participating as a member of the management team. He/she will seek to provide tenants with a safe, positive and healthy living environment.   Catholic Community Services and Catholic Housing Services work together in partnership to provide permanent supportive housing to individuals and families who have experienced homelessness in King County. This program’s success is possible through a robust model of collaboration between Catholic Community Services and Catholic Housing Services.  We work together in partnership to provide permanent supportive housing to individuals and families who have experienced homelessness in King County. We believe that everyone deserves a safe, affordable place to call home. We understand that people experiencing homelessness come to us with complex histories, and that it takes a highly collaborative, creative team to help end the cycle of homelessness permanently. The case manager will have access to motel vouchers for emergent needs while working to secure permanent housing options. The goal will be to engage and build relationships with potential residents of the upcoming Skagit Permanent Supportive Housing Project.   The program will use a Low-Barrier and Housing First approach focused on client centered goals, but will also work with local motels and landlords at scattered-site locations to integrate individuals into housing and to assist with housing stability.  Incumbent must be comfortable engaging with clients in a variety of settings which might include the streets, motels, the hospital, or jail.  Incumbent will track data and report outcomes related to housing status and vulnerability as measured by agreed upon assessment tool and time-frames.   For internal candidates/union positions only: Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.
